In my 2 playthroughs of DOS:EE, I have burned Cassandra's bones, then kill her and I still have not gotten the achievement. What am I not doing right?

After Cassandra was vulnerable, did you speak to her when you returned? Apparently if you attack at a distance the dialogue will not start and the achievement will not be unlocked when she is killed.

Ah, I see, what I did was I gave Arhu the quest item which initiates the fight. Thank you, now I know to talk to Cassandra first.
