Does playing coop where everyone is off doing their own thing mean that everyone misses out on a lot of stuff?

For instance, can you do a quest that someone else has already completed?

Can one person be off in a fight while another is running around town? Is the battle difficulty then scaled down?

Or does the game more or less encourage a team to stay together for most things?

If someone in your party split off and completes a quest. It completes for everyone else. All the quests are shared, but I'm not sure about the premade Origin character quest lines in co-op.

Yes, one person could be in town selling stuff and another could be on the other side of the map fighting a battle by themselves concurrently.

The battle difficulty does not scale down or up.

Generally yes, if you want to experience everything with everyone at the same time. It is entirely possible to beat the current act 1 with a single party member only, so don't think you must always be together.
I believe the separating away from the main party feature is mostly just for fun and it allows for some role playing or for people who wants a different experience playing the game. It's all up to you and your friends how you want to experience it yourself.
